There is no right answer for every practice. The usual tradeoffs:
Utah's birth rate keeps obstetric practices in Salt Lake and Davis counties busier than average, and the billing question is rarely about the delivery itself. It is about who attended it. Practices that employ certified nurse midwives alongside physicians need their midwives credentialed with each plan, and a delivery by a midwife credentialed with one plan and not another produces a claim that goes nowhere. Plans that credential midwives separately often want the collaborating physician named on the claim.
Group practices also share deliveries on call, so the physician who did the delivery may not have done any prenatal care. Global billing then has to be split, with the prenatal portion going to whoever saw her and the delivery to whoever caught the baby. Many billers handle it by guessing. A group that runs a birth center or does home visits has a different claim again. Ultrasounds in the office carry frequency limits on many plans, so a third scan without a documented reason often goes unpaid.
